Biliary papillomatosis is a lesion characterized by multifocal papillary tumours which has the potential of local recurrence. There is usually cytological atypia. Therefore it is difficult to differentiate it from papillary carcinoma. Sometimes malign transformation occurs. Our case is a sixty eight years old man who had papillomatosis all over the gallbladder and multifocal intramucosal carcinoma and adenocarcinoma that arised from this lesion. We discuss this rare case reviewing the literature.
