Urogenital tuberculosis in males

We conducted a retrospective study on clinicopathologic aspects of ten tuberculosis cases located in prostate, urinary bladder, epididym, and testis. The disease was located in prostate in three cases, prostate-bladder in two cases, bladder in two cases, epididym in two cases, and epididym-testis in one case. Histopathologic examination revealed granuloma formation and Langhans type giant cells in all cases. Caseous necrosis was present in five cases (50 %) and no tuberculous bacilli could be detected by acid-resistant bacilli (ARB) stain in the sections of the cases. One of the cases was negative by ARB stain, one was negative by both staining and culture of urine, and one case was also negative by PCR. However, histopathologic examination revealed chronic granulomatous inflammation suggesting the presence of tuberculosis. Diagnosis of these three cases were verified by excellent response to antituberculous therapy. In conclusion, the patient should be carefully evaluated by his/her clinical, radiologic, and microbiologic findings. In the case of negative ARB in urine in spite of clinical suspect, PCR and biopsy would be helpful.
