Fibro-osseous reparative lesion of rib is an unusual benign lesion. All twenty-four cases reported so far in the literature were adults. Most of the patients were asymptomatic and the lesions have been discovered incidentally by bone scans performed in order to rule out metastatic disease. The lesion generally has areas resembling fibrous dysplasia and osteoid osteoma. The presented case is a 61-year-old man who had a solitary lesion on the 10th rib detected by bone scan. It was considered as a tumor of the rib and resection was performed. Histologically, the lesion was characterized by a central foamy, sheet-like core of histyocytes and woven bone trabeculae around them. The metaplastic bone trabeculae residing with in the fibrous stroma exhi-bited a zonal pattern of maturation toward lamellar bone at the periphery of the lesion. Fibro-osseous reparative lesion of rib should be considered in the differential diagnosis, whenever a solitary lesion in the rib has been discovered in patients investigated for metastatic disease.
