Comparative Study of Core Needle Biopsy and Fine Needle Aspiration Cytology in Palpable Breast Lumps: Scenario in Developing Nations

Objective: The purpose of this study was to evaluate the utility of core needle biopsy as a diagnostic tool for palpable breast lumps in developing countries as compared to fine needle aspiration cytology. Material and Method: All patients attending the surgery outpatient department with palpable breast lumps were subjected to fine needle aspiration cytology and core needle biopsy by the same operator in a single session. Fine needle aspiration cytology was performed by the standard technique. Core needle biopsy was done freehand using a 14G manual core biopsy needle. Reporting categories of the two techniques were taken from the standard National Health Service Breast Screening Programme criteria and were compared with the final histopathology results. Results: A total of 107 patients underwent fine needle aspiration cytology and core needle biopsy simultaneously. Histopathology was available for 85 cases. Statistical analysis of fine needle aspiration cytology and core needle biopsy showed no significant difference between the diagnoses offered by core needle biopsy and histopathology while there was a significant difference between fine needle aspiration cytology and histopathology diagnoses. Conclusion: Core needle biopsy detected more breast carcinomas as compared to fine needle aspiration cytology with a sensitivity 95.83% as opposed to 64.58%. Though both the techniques were equally specific (100%), Core needle biopsy was able to correctly categorize borderline / inadequate lesions into definitely benign and malignant categories. We suggest that core needle biopsy should be preferred over fine needle aspiration cytology for the diagnosis of palpable breast lumps with fine needle aspiration cytology being reserved for definitely benign lesions.

The Relation of Parafibromin Expression with Clinicopathological Factors in Invasive Breast Carcinomas

Objective: We aimed to find the relationship between parafibromin expression and clinicopathologic variables of breast carcinoma. Material and Method: Ninety-seven cases of invasive breast carcinoma diagnosed at our department between the years 2010–2013 were included in the study. The parafibromin expression state was compared with the estrogen receptor, progesterone receptor, cerbB2, Ki67 results, and the clinicopathological variables. Results: Among 97 breast carcinoma cases, 66 (68%) were invasive ductal carcinoma. The average age was 54.3 (min:25, max:100), and the average tumor size was 31.1 mm (min:7, max:120). Lymph node metastasis was detected in 58% of the cases. Eleven were diagnosed with metastasis amongst 77 cases whose distant metastasis data could be reached. Eleven cases were lost due to breast carcinoma. As the tumor grade increased, the possibility of distant metastasis and lymph node metastasis increased as well (p=0.04, p=0.05, respectively). The mean follow-up duration of the cases was 26.6�}9.8 (min. 6, max. 53) months, and there was no significant difference in survival between the other variables. Of the cases, 21.6% were negative, 9.3% were (+) positive, 11.3% were (++) positive and 57.7% were (+++) positive for parafibromin. It was found that there was an inverse correlation between the Ki67 proliferation index and lymph node metastasis and the parafibromin expression (p=0.018, p=0.029, respectively). Conclusion: We suggest that parafibromin may be a possible prognostic and predictive parameter for breast carcinomas. As the data on this matter in the literature is limited, it would be beneficial to investigate the matter and evaluate its relationship with survival in larger series.

Twist Expression in Pleomorphic Adenoma, Adenoid Cystic Carcinoma and Mucoepidermoid Carcinoma of Salivary Glands

Objective: Twist is an important transcription factor that induces epithelial-mesenchymal transition and therefore plays an important role in tumor progression. There are a few studies on Twist expression in salivary gland carcinomas. The aim of this study was to evaluate and compare the expression of Twist in the most common benign and malignant salivary gland tumors and to determine whether its expression was correlated with any tumor characteristics. Material and Method: In this retrospective cross-sectional study, 45 cases including 11 cases of normal salivary gland, 12 pleomorphic adenomas, 12 adenoid cystic carcinomas and 10 mucoepidermoid carcinomas were enrolled. The mean and intensity of Twist expression were evaluated immunohistochemically and were compared using statistical analysis. Results: The expression of Twist was higher in malignant salivary gland tumors in comparison with normal glands and benign tumors (p= 0.03). It was also higher in pleomorphic adenomas in comparison with normal tissue. Adenoid cystic carcinomas and mucoepidermoid carcinomas showed no significant difference in Twist expression (p= 0.50). There was no correlation with the size, stage or grade of tumor. Conclusion: The findings showed that Twist might play a role in the formation of salivary gland neoplasm and also may affect malignant transformation and tumoral invasion. The exact mechanism of this marker and the possibility of using it as a therapeutic target require further investigation.

Comparative Study of Mast Cell Count in Oral Reactive Lesions and Its Association with Inflammation

Objective: Reactive hyperplastic lesions are one of the most common lesions of the oral cavity. Mast cells can be found in various oral lesions. However, the exact role of mast cells in oral reactive lesions is not fully understood. The aim of this study was to compare the mast cells count in various oral reactive lesions and to evaluate the correlation between the mast cells number and the intensity of inflammation. Material and Method: This cross-sectional study was performed on 80 samples of oral reactive lesions including irritation fibroma, pyogenic granuloma, peripheral giant cell granuloma and peripheral ossifying fibroma (20 for each). Eight samples of normal oral mucosa were selected as the control group. To determine the mast cells count and the intensity of inflammation, Toluidine blue and Hematoxylin & Eosin staining was performed respectively. The data was analyzed with SPSS (V.21), Kruskal-Wallis, Tukey and Spearman's correlation coefficient tests. Results: In this study, a significant increase in mast cells count was found in oral reactive lesions compared to normal oral mucosa (p<0.001). Difference of mast cell count between irritation fibroma and the peripheral giant cell granuloma groups (p=0.023), peripheral ossifying fibroma and peripheral giant cell granuloma groups (p=0.001) was statistically significant. There was a significant correlation between mast cell count and the intensity of inflammation in the irritation fibroma and pyogenic granuloma groups. Conclusion: The finding of this study showed that mast cells count in peripheral ossifying fibroma and irritation fibroma were significantly higher than normal oral mucosa, so mast cells may have some role in the induction of fibrosis in oral reactive lesions.

A Critical Appraisal of the Spectrum of Image Guided Fine Needle Aspiration Cytology: A Three Year Experience from a Tertiary Care Centre in Delhi

Objective: Image guided fine needle aspiration cytology (FNAC) is a safe, simple, rapid, cost effective and accurate modality in evaluation of deep seated lumps (abdominal, pelvic and intrathoracic masses) which are unapproachable by blind FNAC. In this study, a critical analysis of the utility of image guided FNAC in deep seated and otherwise inacessible mass lesions was carried out. Material and Method: Fifty-four ultrasound (USG) or computed tomography (CT) guided FNACs from lumps at any location in the body, that is, thoracic, abdominal, pelvic etc over a period of 3 years (January 2012- December 2014) were included in the study. The most common site aspirated, the indications for a guided aspiration and the accuracy of aspiration cytology in comparison to the histological diagnosis were evaluated, wherever possible. Results: Fifty-four image guided FNACs were performed (46 USG guided and 8 CT guided). Inconclusive, inflammatory, benign tumors, suspicious and malignant lesion rates were 5.6%, 29.6%, 11.1%, 5.6%, and 48.1%, respectively. The most common site was the liver (25.5%), followed by the thyroid (18.5%), lung (14.8%), gallbladder (11.1%), and lymph nodes (7.4%). Cytohistological correlation could be performed in 32 cases with a diagnostic accuracy of 100%. Conclusion: FNAC coupled with a diagnostic imaging technique such as USG or CT can improve the diagnostic yield and a definite diagnosis can be reached in most of the cases.

Salivary Gland Tumors: A 15- year Report from Iran

Objective: The aim of this study was to document the clinicopathologic characteristic of salivary gland tumors in Tehran, Iran, over a 15-year period. Material and Method: A retrospective study was conducted on salivary gland tumors diagnosed at two pathology centers of Shahid Beheshti University of Medical Sciences from March 2000 to March 2015. Patient age, sex, tumor site and frequency, as well as clinical and radiographic features and histopathologic diagnosis constituted the main analysis outcome measures. Results: Of the 45429 biopsies conducted over 15 years, 6065 (13.3%) cases were oral and maxillofacial lesions and 937 (15.4%) of these had tumoral diagnoses. Of the 937 tumoral cases, 184 (19.6%) were salivary gland tumors and among 184 cases, 65 (35.3%) were benign and 119 (64.7%) were malignant. Pleomorphic adenoma was the most frequently occurring tumor, comprising 32.6% of all tumors, followed by mucoepidermoid carcinoma (27.1%) and adenoid cystic carcinoma (22.2%). Tumors were frequently reported in minor salivary glands (75%), particularly in the palate with 89 (48.4%) cases. The peak ages of incidence were the fourth and sixth decades of life. Malignant salivary gland tumors showed a predilection for females (72.9%), which was statistically significant (P<0.01). Conclusion: The data presented herein are similar to previously published reports in other countries and other areas of Iran. However, some differences were observed in our study, such as higher overall frequency, a lower mean age of patients with malignant tumors, and the particular sites of involvement. These differences can be attributed to racial factors, the pathology centers of sample collection, and the duration of the studies.

Natural Killer-Like T-Cell Lymphoma Localized to the Terminal Ileum: Case Report

Intestinal intraepithelial lymphocytes are non-organized lymphoid populations that are composed of heterogeneous subsets with diverse ontogeny and phenotypes, and the differential diagnosis is crucial. A 43-year-old male patient underwent an emergency laparotomy due to a perforated mass of the terminal ileum. A right hemicolectomy plus small bowel resection was performed. Histopathological examination showed medium to large cells with vesicular nuclei, including marked nucleoli with large, colorless cytoplasm. No signs of celiac disease were found in the adjacent mucosa. The tumor cells were immunohistochemically CD45+, CD3+, CD4+, CD8+, CD56+, Pan-Cytokeratin-, CD20-, CD79a-, CD5- and CD30-. Endomysial antibody and antigliadin antibody, IgM and IgG tests; and anti-Ebstein Barr virus latent membrane protein all proved negative. Finally, the histopathological diagnosis of tumor mass was natural killer-like T-cell lymphoma. Primary intestinal cytotoxic natural killer-like T-cell lymphoma is a rare entity, which is difficult to distinguish from other T-cell lymphomas. In addition to microscopic evaluation, immunohistochemical analysis and serological tests are essential to reach a definitive diagnosis.

Broad Ligament Perivascular Epithelioid Cell Tumor (PEComa) of Uncertain Malignant Potential

PEComas are uncommon mesenchymal tumors often involving the pelvic organs. They have an unpredictable behavior. Accurate diagnosis and long-term follow-up is therefore essential in these patients. We report this case of PEComa of uncertain malignant potential in an unusual location with excellent prognosis.

Incidentally Discovered Extensive Squamous Metaplasia within Borderline Phyllodes Tumor: Presentation of a Rare Tumor

Phyllodes tumors are uncommon biphasic fibroepithelial neoplasms of breast, comprising less than 1% of all breast neoplasms. We therefore aimed to present the case with its microscopic findings. In this article, we report a 59-year-old female admitted to the general surgery department with a rapidly, enlarging, palpable mass in right breast. After histopathological examination, it was diagnosed as borderline phyllodes tumor with extensive squamous metaplasia. Metaplastic changes are infrequent in the stromal and epithelial component of these tumors. Extensive squamous metaplasia within phyllodes tumor is rare and may occur in benign, borderline and malign subtypes.

Clinical and Pathological Findings on Intoxication by Yellow Phosphorus After Ingesting Firework Cracker: A Rare Case of Autopsy

Yellow phosphorus is a toxic substance used in the production of firework cracker, fireworks, ammunition and agricultural dung. When ingested, it shows its effects mainly in the liver, the kidneys, and the brain. A four-year-old girl had died as a result of acute hepatic failure caused by ingesting a firework cracker. The case showed high levels of hepatic enzymes, along with non-specific signs such as nausea, vomiting and diarrhea. Autopsy revealed diffuse microvesicular steatosis in the liver and disseminated degeneration in the proximal tubules of the kidneys. In cases with concomitant hepatorenal failure and cardiovascular collapse, death is inevitable. However, when only hepatic failure develops, hepatic transplantation may be lifesaving. Although intoxication from ingesting yellow phosphorus has a very high rate of mortality, forensic cases are extremely rare in the literature.

Concomitant Aspergillus Species Infection and Squamous Cell Carcinoma Diagnosed on Pap Smear

Concomitant infection with Aspergillus species and cervical squamous cell carcinoma in the female genital tract is a rare occurrence and attributed to the opportunistic nature of infection in the immunocompromised state due to the underlying malignancy. The contamination of smears with Aspergillus species should be excluded. The diagnosis of Aspergillus species infection along with squamous cell carcinoma was established on cervicovaginal pap smears in a 62-year-old female presented to gynecological clinic with complaints of stress urinary incontinence. Speculum examination revealed first-degree cervical descent. Smears showed features of squamous cell carcinoma along with fungal spores and fruiting body with hyphae of Aspergillus species. The presence of fruiting bodies and hyphae of Aspergillus species with coexisting squamous cell carcinoma is rare in routine pap smears. True infection needs to be distinguished from contamination by Aspergillus species. Early diagnosis can be established on routine cervicovaginal Pap smear examination.

A Gossypiboma (Textiloma) Mimicking a Gastrointestinal Stromal Tumor: A Case Report

Gossypiboma (textiloma) is a rare but preventable complication occurring due to oblivion of the surgical material during an operation that carries important diagnostic difficulties as well as high morbidity and mortality risks. The actual incidence is unknown because of its medicolegal aspects. Our case is a 36-yeas-old female patient, who had a caesarean section two years ago in another center. Clinical examination and radiological investigations gave the impression of “gastrointestinal stromal tumor” and the surgically taken out mass was sent to our pathology department. The case was reported as a “gossypiboma”' and presented here to emphasize the diagnostic difficulties in the preoperative period, the life-threatening nature of the condition and as well as the rarity of the reported cases.
