A case of ovarian endometrioid adenofibroma was presented which was detected during pathologic examination of total abdominal hysterectomy and bilateral salpingo-oophorectomy of a 43 year old woman that was performed for pelvic mass/ uterine fibroid. Histologic examination revealed tubuler glands lined by endometrioid epithelium in an ample amount of fibroblastic stroma, some of the glands were cystically dilated. Squamous metaplasia was encountered at the glandular epithelium in some areas. Histologic features of atypical proliferative endometrioid tumor, malignant endometrioid adenofibroma and müllerian adenosarcoma which should be encountered in the differential diagnosis of this rare lesion were evaluated with the literature findings.
