A case of sarcomatoid carcinoma in a 75-year-old male with epiglottic location was presented. Histological examination of the tumor revealed rhabdomyosarcoma and osteosarcoma-like areas admixed with a minor component of squamous cell carcinoma. The rhabdomyosarcomatous component showed strong vimentin, desmin and myoglobin immunoreactivity. Histologically most of these tumors consisted of conventional squamous cell carcinoma and sarcomatoid component which frequently arranged in a storiform pattern. A combination of extensive osteosarcomatous and rhabdomyosarcomatous component has been reported in rare cases. A diagnosis of laryngeal sarcomatoid carcinoma requires considerable caution when the sarcomatous portion predominates and care must be taken not to overlook the carcinomatous areas.
